The school health program consists of three interdependent and inter-related parts: health education, a healthy environment and health services. The goal of the Montville Public Health Program is to assist each child to attain and maintain his/her optimum state of health so that he/she may benefit to the maximum degree from his/her educational experience. Therefore, the health status of each student is monitored to help identify and address unmet needs of each child. The services provided through the Health Office are defined by the National Association of School Nurses as well as the School Health Unit of the Department of Public Health in Connecticut.

 

The following are services provided by the school nurses in the Montville Public School District:

 

Provides direct health care to students and staff

 

Provides leadership for the provision of health services

 

Provides screening and referral for health conditions

 

Promotes a healthy school environment

 

Promotes health

 

Serves in a leadership role for health policies and promotion

 

Serves as a liaison between school personnel, family, community, and health care providers

DTaP vaccine is not given on or after the 7th birthday and may be given for all doses in the primary series.
Tdap can be given in lieu of Td vaccine for children 7 years and older unless contraindicated. Tdap is only licensed for one dose.

Hib is not required for children 5 years of age or older.

Pneumococcal is required for all PreK and K students less than 5 years of age.

Hep A requirement for school year 2017-18 applies to all PreK, K, 1st , 2nd , 3rd , 4th & 5th graders born 1/1/07 or later.

Hep B requirement for school year 2017-2018 applies to all students in grades K-12. Spacing intervals for a valid Hep B series: at least 4 weeks between doses 1 and 2; 8 weeks between doses 2 and 3; at least 16 weeks between doses 1 and 3; dose 3 should not be given before 24 weeks of age.

Second MMR for school year 2017-2018 applies to all students in grades K-12.

Meningococcal Conjugate requirement for school year 2017-18 applies to all students in grades 7-12
Tdap requirement for school year 2017-18 applies to all students in grades 7-12
If two live virus vaccines (MMR, Varicella, MMRV, Intra-nasal Influenza) are not administered on the same day, they must be separated by at least 28 days (there is no 4 day grace period for live virus vaccines). If they are not separated by at least 28 days, the vaccine administered second must be repeated.

Lab confirmation of immunity is only acceptable for Hep A, Hep B, Measles, Mumps, Rubella, and Varicella.
VERIFICATION OF VARICELLA DISEASE: Confirmation in writing by a MD, PA, or APRN that the child has a previous history of disease, based on family or medical history.

If you are unsure if a child is in compliance, please call the Immunization Program at (860) 509-7929. New Entrant Definition: *New entrants are any students who are new to the school district, including all preschoolers and all students coming in from Connecticut private, parochial and charter schools located in the same or another community. All preschoolers, as well as all students entering kindergarten, including those repeating kindergarten, and those moving from any public or private preschool program, even in the same school district, are considered new entrants. The one exception is students returning from private approved special education placements – they are not considered new entrants.

Montville Integrated Preschool at Oakdale Elementary School

To be eligible for the Montville Integrated Preschool, the following documentation is required at the time of registration:

1. Proof of Residency – Copy of one of the following: mortgage deed or current valid lease.

2. Birth Certificate or Baptismal Record

3. Physical Examination Form (yellow) completed and signed by your child's doctor, date of examination should be within one year of date of entering preschool. Parents must also complete Part I of the Physical Examination form.

4. Proof of Immunization
Immunization Requirements for Preschool:

a. DTP/Td-minimum of four (4) doses of diphtheria, pertussis, and tetanus, three (3) doses given at a minimum of four (4) week intervals followed by a fourth dose at least six (6) months after the third dose.

b. TOPV/IPV-minimum of three (3) doses of polio vaccine – two (2) doses given at least four (4) weeks apart and a third dose given at least two (2) months after the previous dose.

c. MMR - measles, mumps and rubella; one (1) dose given on or after the first birthday.

d. HIB - Hemophilus influenza, type B, minimum of one (1) dose given on or after the first birthday.

e. Hepatitis B - completion of a series of three (3) doses of Hepatitis B vaccine, two (2) doses given at least four (4) weeks apart, followed by a third dose at least sixteen (16) weeks after the first dose, and at least eight (8) weeks after the second dose, and the third dose shall be given no earlier than twenty-four (24) weeks of age.

f. Varicella – one (1) dose given on or after the first birthday.

g. Hepatitis A - two (2) doses given at least six (6) months apart, the first dose given on or after the first birthday.

h. Pneumococcal disease – one (1) dose of pneumococcal conjugate vaccine given on or after the first birthday.

i. Influenza (flu): one (1) dose given annually between August 1 and December 31. PRESCHOOL STUDENTS WILL BE EXCLUDED FROM SCHOOL BETWEEN JANUARY 1ST AND MARCH 31ST (FLU SEASON), IF THEY HAVE NOT RECEIVED A FLU VACCINATION.

If your child has never before had a flu vaccination, he/she will need two (2) doses, at least four (4) weeks apart.

Preschool students enrolling after March 31 are not required to receive a flu vaccine for the current school year.
